Dear colleagues

I am writing to you today as a family doctor deeply committed to the values we profess, drawing on my significant history and involvement with WONCA.

As you know I have been an ardent supporter for the development of Family Medicine in South Africa and Africa. I served as President of the African Region of WONCA (2018-2021), overseeing achievements such as growth in member organisations, strengthening relationships with WHO Afro, developing the African Forum for Primary Health Care (AfroPHC). I contributed strongly to the WONCA Executive Committee, including Chairing the Bylaws Committee. This continued when elected as Member-at-large on WONCA Executive (2021-2023). I chose not to stand for re-election in 2023 due to work constraints. I wanted to stand at the Lisbon Conference 2025, but I decided against it. This was a conscious decision because, for me, addressing the critical issue of Palestine and urging WONCA to act consistently on its stated principles is currently more important than being constrained by an elected position within the organisation.

I am writing to you to express my profound concern regarding WONCA’s response to the unfolding catastrophe in Palestine. While WONCA has stated its mission includes improving quality of life, respecting universal human rights, and promoting equity, its public stance and actions specifically regarding Palestine are widely perceived as lacking the specificity and urgency demonstrated in response to other crises, such as the situation in Ukraine. WONCA issued several detailed statements concerning Ukraine, including calls for solidarity, urging members to take action and support, and naming specific humanitarian relief organisations. In contrast, statements regarding the Israeli and Palestinian crisis have been perceived as less robust and lacking the same level of detail or specific calls to action. This perceived disparity leads to accusations of a “double standard”.

Sources highlight the severity of the humanitarian situation in Palestine, particularly in Gaza and the West Bank, noting its devastating impact on healthcare infrastructure and personnel. Actions on the ground have been described using terms like “apartheid,” “illegal occupation,” “genocide,” and “war crimes” by various commentators and sources. The period of 2022-2024 has seen the highest number of attacks against health personnel globally, making their protection in places like Palestine critically urgent. The International Court of Justice (ICJ) ruled in January 2024 that Israel is plausibly committing genocide in Gaza and in July 2024 that Israel has imposed a regime of apartheid in the occupied territories. As doctors, we are bound by medical ethics, integrity, and equality, which emphasise the sanctity of human life, the duty to alleviate suffering, and impartial care.

I urge you to reflect on the challenge this inconsistency poses to WONCA’s credibility and moral authority. Not upholding universal human rights consistently, particularly in the face of potential crimes under international law, risks creating a “tainted response” that rings hollow and hypocritical.

I strongly urge WONCA to consider demonstrating more specific and principled action regarding Palestine. This could involve actions such as those outlined here: https://profmoosa.com/family-doctors-for-global-justice/campaign-palestine/action-for-palestine/, which proposes ways to urge WONCA leadership and explore resources.

Specific requests outlined include calling on WONCA (including WONCA Europe, WONCA Working Parties and Special Interest Groups) and the Israeli Association of Family Physicians (IAFP) to issue statements condemning actions against healthcare workers, facilities, and civilians, considering the suspension of IAFP membership if they do not issue such a statement, moving the IAFP to the WONCA East Mediterranean region, disinviting representatives from Israeli medical and academic institutions who have not publicly condemned the actions, dedicating the South African WONCA conference in 2027 to rebuilding healthcare in Palestine, and creating initiatives like scholarships for Palestinian doctors to continue their training. This mirrors the creativity and support demonstrated for Ukraine in 2022.

This issue, described by some as a settler colonial project with clear history and OECD complicity, is not “complicated”. It is simple! It is time for WONCA to take a clear stance, reflecting the same level of support for the people and healthcare system of Palestine as it did for Ukraine.

I will also be writing to colleagues across the globe, asking them to join the Family Doctors for Global Justice Caucus and to support this call for consistent and principled action, culminating hopefully to some action by the Council in Lisbon 2025 but definitely in motions all the way to Cape Town 2027. Our silence for all these years will easily be viewed as complicity in genocide, and I refuse to let WONCA, and the community of family doctors, be damned to that!

Estimados colegas:

Hoy les escribo como médico de familia, profundamente comprometido con los valores que profesamos, basándome en mi amplia trayectoria y participación en WONCA.

Como saben, he sido un ferviente defensor del desarrollo de la Medicina Familiar en Sudáfrica y África. Fui Presidente de la Región Africana de WONCA (2018-2021), supervisando logros como el crecimiento de las organizaciones miembros, el fortalecimiento de las relaciones con la OMS Afro y el desarrollo del Foro Africano de Atención Primaria de Salud (AfroPHC).

Contribuí significativamente al Comité Ejecutivo de WONCA, incluyendo la presidencia del Comité de Estatutos. Continué en mi cargo tras ser elegido Miembro General de la Junta Directiva de WONCA (2021-2023).

Decidí no presentarme a la reelección en 2023 debido a limitaciones laborales. Quería presentarme a la Conferencia de Lisboa de 2025, pero decidí no hacerlo. Fue una decisión consciente, ya que, para mí, abordar la crucial cuestión de Palestina e instar a WONCA a actuar consecuentemente según sus principios declarados es actualmente más importante que estar limitado por un cargo electo dentro de la organización.

Le escribo para expresar mi profunda preocupación por la respuesta de WONCA a la catástrofe que se está desatando en Palestina. Si bien WONCA ha declarado que su misión incluye mejorar la calidad de vida, respetar los derechos humanos universales y promover la equidad, su postura pública y sus acciones, específicamente con respecto a Palestina, se perciben ampliamente como carentes de la especificidad y urgencia demostradas en respuesta a otras crisis, como la situación en Ucrania. WONCA emitió varias declaraciones detalladas sobre Ucrania, incluyendo llamamientos a la solidaridad, instando a los miembros a actuar y apoyar, y nombrando organizaciones de ayuda humanitaria específicas.

En cambio, las declaraciones sobre la crisis israelí y palestina se han percibido como menos contundentes y carentes del mismo nivel de detalle o llamamientos específicos a la acción.

Esta disparidad percibida da lugar a acusaciones de "doble moral". Diversas fuentes destacan la gravedad de la situación humanitaria en Palestina, en particular en Gaza y Cisjordania, y señalan su devastador impacto en la infraestructura y el personal sanitario.

Diversos comentaristas y fuentes han descrito las acciones sobre el terreno con términos como "apartheid", "ocupación ilegal", "genocidio" y "crímenes de guerra".

El período 2022-2024 registró el mayor número de ataques contra el personal sanitario a nivel mundial, lo que hace que su protección en lugares como Palestina sea crucial.

Les insto a reflexionar sobre el desafío que esta inconsistencia supone para la credibilidad y la autoridad moral de WONCA.

No defender los derechos humanos universales de forma consistente, en particular ante posibles crímenes de derecho internacional, corre el riesgo de crear una respuesta contaminada que suene hueca e hipócrita.

Insto encarecidamente a WONCA a que considere la posibilidad de tomar medidas más específicas y basadas en principios con respecto a Palestina.

Las solicitudes específicas descritas incluyen instar a WONCA (incluyendo WONCA Europa, los Grupos de Trabajo de WONCA y los Grupos de Interés Especial) y a la Asociación Israelí de Médicos de Familia (IAFP) a emitir declaraciones condenando las acciones contra el personal sanitario, los centros de salud y la población civil; considerar la suspensión de la membresía de la IAFP si no emiten dicha declaración; trasladar la IAFP a la región del Mediterráneo Oriental de WONCA; retirar la invitación a los representantes de instituciones médicas y académicas israelíes que no hayan condenado públicamente las acciones; dedicar la conferencia sudafricana de WONCA de 2027 a la reconstrucción de la atención sanitaria en Palestina; y crear iniciativas como becas para que los médicos palestinos continúen su formación.

Esto refleja la creatividad y el apoyo demostrados a Ucrania en 2022.

Este asunto, descrito por algunos como un proyecto colonial con una historia clara y complicidad de la OCDE, no es "complicado". ¡Es simple!

Es hora de que WONCA adopte una postura clara, que refleje el mismo nivel de apoyo a la población y al sistema sanitario de Palestina que el que mostró a Ucrania.

También escribiré a colegas de todo el mundo para pedirles que se unan al Caucus de Médicos de Familia por la Justicia Global y que apoyen este llamado a una acción coherente y basada en principios, que culmine, ojalá, en alguna acción del Consejo en Lisboa en 2025, pero definitivamente en mociones hasta Ciudad del Cabo en 2027. 

Nuestro silencio durante todos estos años fácilmente se interpretará como complicidad en un genocidio, y me niego a permitir que WONCA y la comunidad de médicos de familia se vean obligados a ello.
